Online Enrollment Form!
You can complete a paper enrollment form. Please return this with the other forms.
Must be a CLEAR, current (not expired) MA. driver's license, MA. ID or US Passport. We can accept emailed smart-phone photos of an ID.
We then open an account we can co-manage to assist in paying rent and any other bills!
When the account is open we contact you with the information. You contact SSA or your employer and arrange for direct deposit!
Please be as thorough as possible! If you need to gather more information, you can save the form and return to finish for up to two weeks!
We prefer online enrollment forms, but if you complete a paper enrollment please return this with your other forms!
The Enrollment Verification Forms and Power of Attorney *required, MUST be notarized prior to returning to us! Always send us the originally signed and notarized forms!
